• Forms. Please note that if the form requires your information or signature, please fill this out BEFORE sending us the document.

• Please ensure your name is on the document; otherwise, we won't know who it's from!

 

There are two options to provide us with files:

• Please note that you will see a checkmark as in the image below when your file has been successfully uploaded.  To protect patient privacy, you will not be able to view the documents after you have uploaded them.

• If you don't have a scanner, please use an app like Adobe Scan on your mobile device to send us a high-quality document.  It is available for both Apple and Android.

• If you don't have an upcoming appointment already scheduled, please call us at 416-751-0465 to confirm receipt of your document and plan any necessary next steps
